Default Error message 1146 table does not exist

I haven't customised my osCommerce site much and it seemed to work fine UNTIL, I decided to test it before letting it loose on the public. I created a new customer but when I log in, I get this error:

    1146 - Table 'ictans_osCommerce.TABLE_COUPON_GV_CUSTOMER' doesn't exist

    select amount from TABLE_COUPON_GV_CUSTOMER where customer_id = '4'

    [TEP STOP]

The other strange thing is that when I hover over the Gift Voucher FAQ link, the target shows as: http://www.mysite.co.uk/catalog/FILENAME_GV_FAQ
which obviously doesn't exist. Before I delete everything yet again, can anyone help? PLEEEEEZE!

did you run the install script? Looks like some files are missing and the DB is not setup correctly...

Yes, ran it as per instructions and everything installed OK. According to phpMyAdmin, the table exists so I don't think it cxan be a DB problem. I don't actually need the gift voucher facility anyway. How can I turn it off? Maybe this will cure the problem.

Check your includes directory. You need this file:
    catalog/includes/add_ccgvdc_application_top.php

That is where the TABLE_COUPON_GV_CUSTOMER is defined.
