Orders: Province in Albania posted for invoice instead of correct Canada/US province/state

I'm getting some odd orders lately - the customers address is correct when I go to the actual Customers listing, however the invoice chooses a province in Albania instead of Canada or the USA. This only happens about once every couple of weeks...today the invoice was saying Tropoje, Canada instead of Alberta, Canada. Customer didn't catch it, not sure if he even noticed.

A pre-existing customer, prior to an osCmax upgrade?
This can happen due to the addition of many more countries/counties compared to older osCmax versions. New customers will get the correct county/state/zone but not existing customers. (This is an ongoing issue that I'm trying to get my puny brain around.)

This doesn't sound exactly like your scenario but does indicate a problem between the country and zones relationship/database tables.

Nope not old customer, in both recent cases a new customer.
I have been running the same version of oscMax (2.5.4) since it came out a couple of years ago. Current PHP is 5.6.23, Apache 2.4.29, MySQL 5.0.11

A completely different tack then, assuming that you've checked the zones table to make sure ones with the Canada country ID are correct.
If you run Apache with mod_sec, look for any triggers relevant to osCmax URLs and disable any highlighted rule(s). I've had mod_sec play havoc with stores and it took a while to look there.

I haven't touched the Apache settings, and (in fact) don't know how to implement mod_sec. So can't imagine that is the problem. Went through the Server Information and could not find anything there about Apache settings.

Further to Albania, I have Albania's Status set to Inactive (along with a few other countries - in Admin|Locations/Countries), so theoretically it should not allow Albania to be selected. Problem hasn't recurred, and may not show up for another month or so.
